How to Upgrade Items in Pokemon GO

The Upgrade is one the most valuable items in Pokemon GO. It allows trainers to evolve Porygon into Porygon2 and gives them the chance to progress in their Pokedex.

This item is scarce, but can be found by spinning a Photo Disc at a Pokestop or as a prize for special events. What does this item actually do?
